Caregiver Services: Senior Care Assessment & Care Plan

When an older adult or caregiver needs answers, service, support or counseling, a Senior Care Assessment is the best way to start.  At St. Andrew's our loving staff is qualified to ask the right questions and find the right answers to help during this difficult time.  Our assessment cover key senior care topics such as:

• Does the senior have the appropriate physician?

• Are the nutritional needs of the senior being met?

• Are the senior's medications being managed appropriately?

• Does the aging individual need some in home help?

• Can and should the senior stay in his or her own home?

• What is the difference between assisted living and a skilled nursing facility?  What are the pros and cons of each?

• Are there some assistive devices that would make it safer for the aging individual to get around?

These and many more questions are answered with a senior care assessment. All our assessments are comprehensive and cover all physical, psychological, emotional, environment issues as well as determining the need for community resources.  The result is an assessment with answers for the older adult and the caregiver identifying what is best, safe and appropriate.
