St. Andrew's At Home Services: Caregiver Services (Geriatric Care Management Services)

“Dad lives in a long term care community, but I
can’t be there all of the time. How do I know he
is getting the care he deserves?”

“I am worried that mom isn’t safe living alone. I
just can’t have her move in with me and I feel
very guilty about it. What can I do?”

There are many family members, spouses, friends and neighbors that are worried or concerned about an older adult that is frail and needs some assistance. These individuals are caregivers and St Andrew's offers services designed specifically to provide them with the answers and solutions they need.

An experienced St. Andrew's senior care specialists will visit with families and older adults to assess the caregiving situation.  Services provided include:

Consultation:
A senior care specialists visit with families and older adults to learn first hand about the caregivers concerns and needs.

Assessment:
The specialists assesses the older adult's functional levels, capacity and support systems along with their psychological and emotional well-being.

Caregiver Situational Assessment: 
The senior care specialists will discuss and assess the needs and wants as well as the health and concerns of the person providing the care.

Home Safety Assessment: 
The senior care expert will review the home environment, identify safety issues and provide solutions to address all identified concerns.

Senior Care Plan:
The St. Andrew's Eldercare specialists will develop a detailed plan for the caregiver stating the strengths of the person, outlining concerns and providing interventions to address all issues

Counseling:
Skilled and experienced eldercare specialist offer psychological testing and counseling to support older adults and family members through difficult transitions.

Senior Care Resources:
Available community resources, services and options are identified as well as linkage to a wide range of qualified and certified trusted advisors in various fields, such as eldercare attorneys, financial advisors, senior housing, skilled nursing care facilities, movers, and health care specialist.

Care Coordination:
Appropriate services/resources are coordinated ensuring ease of access for families.

Monitoring:
Changes in the needs and wants of the caregiving and cared for senior are closely monitored and families are always kept informed.

Advocacy:
An eldercare specialist serves as a liaison between health care providers and other community services to ensure care and service provided is appropriate, that questions are answered, and information is accurate.

With an eldercare specialist no one is ever alone. Whether in town or out of town an eldercare specialist is never far away and can be that extra "family" member that provides objective solutions to difficult situations.
